Filling in the patient's personal data
Our patients' personal data is collected primarily on the basis of a medical history questionnaire that you fill out before performing dental hygiene. All data is processed and retained for the period strictly necessary in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data.
For what legal reason is the data processed?
Your personal data is processed for a legal reason and for the purpose of the necessity of processing for the fulfillment of a legal obligation to which *DH is subject, which is set out in particular in Section 53, paragraph 2 of the Health Services Act. In the case of the title, e-mail, telephone number and health insurance company of the patient, your personal data is processed for a legal reason and for the purpose of the legitimate interests of DH in communicating with you as a patient, for subsequent billing information and for the possible financing of the relevant procedure by the health insurance company. Personal data that indicate your health condition are processed for the reason of necessity for the purposes of providing healthcare or treatment.
Who will have access to the data?
The processor of all your personal data is a natural person (hereinafter referred to as the employee) who
processes all your personal data electronically, using special software (program). And this also applies to the relevant dental hygienist. Only an IT company and a payroll accountant who have concluded a personal data processing agreement with this person work with this data. The appendix further elaborates on mutual rights and obligations under the GDPR, the method and purpose of personal data processing, including technical security of personal data, obligations towards data subjects and the Office for Personal Data Protection.
